No, sadly, I don't know think that Sam knows that he's dead, it's not like they're on fluffy white clouds- they're in the impala on an eternal roadtrip...
Also I'd like to think that God would erase all of Sam's guilt and the bad memories that go with it...so basically the Sam at the end of the story- is a grown up mature S1 Sam without all the worry and the guilt. If that makes sense...
